In order to qualify for sales tax exemption in the state of Missouri, you must fill out the Missouri Form 149 and provide it to the seller, me. You do not need to file this form elsewhere, but I am required to keep it for my records to prove your exemption.
There are several ways to fill out this form:
  • Download the file on your computer, open it with a PDF reader or image viewing software. Most PDF reader software will allow you to fill in the empty fields with your information. Make sure you SIGN/TITLE/DATE the bottom of the form. PDF readers usually have a "e-signature" button so you can type in your signature and it will place it in the field in cursive.
  • OR, download the file on your computer and print it, filling out the empty fields by hand. Then you can either scan the filled out form or take a photo of it with your phone (email it to yourself so you can open it on your computer) then upload it to the submission form below
  • OR, download the file on your computer and print it, then mail the completed form to the following address: STONEHOARDER. 1229 UNION AVENUE, KANSAS CITY, MO, 64101.
